FREE ANTI-RABIES VACCINE

Requirements:
  • Prescription/Resita with doctor’s signature and license #
  • Barangay Indigency of immediate relative
  • Valid ID of the claimant

MEDICINE ASSISTANCE

Requirements:
  • Prescription/Resita with doctor’s signature and license #
  • Medical Certificate
  • Barangay Indigency of immediate relative
  • Valid ID of the claimant

HOSPITALIZATION

Requirements:
  • Medical Abstract or Medical Certificate
  • Hospital Bill
  • Barangay Indigency of immediate relative
  • Valid ID of the claimant

LABORATORY

Requirements:
  • Laboratory Request
  • Medical Certificate
  • Barangay Indigency of immediate relative
